Honoree Nomination Form
The purpose of the Corporate Directors Forum is to help Directors be
better Directors. Our annual Director of The year (DOY) Awards event supports that endeavor.
The 2007 Director of the Year Awards will be held on Tuesday
evening, September 25, 2007 at the Manchester Grand Hyatt at 1 Market Place in Downtown San Diego.
The Purpose of the DOY Awards is to recognize Directors for outstanding
performance in the boardroom and behind the scenes.
Qualifications for nominees

Nominees may be from any industry

Nominees must be a resident of the San Diego area or serve on a board of a company located in San Diego
-
Awards are based on Director's recent achievements (except for the Lifetime Achievement Award)
Note: Past Honorees
have been recognized for their contributions in areas such as:

Corporate Governance
-
Companies in Transition (Change Management contributions,
e.g. start-up, M&A, Management changes)
-
Enhancement of Economic Value (Operational Effectiveness,
e.g. enhanced profitability via better deployment of resources)
-
Corporate Citizenship
-
Lifetime Achievement

Please
consider the Directors with whom you serve and nominate the
highest achievers among the group. A description of the Director's
specific accomplishments that led to your decision
to nominate him/her is required to facilitate final judging
(see space below). Please note that while we are focused on the
single highest performer on a given board, you are not limited
to a single nominee should you sit on multiple boards
A representative of the nominating committee will contact you for references and other
information to support your nomination.
